Delmar Pediatrics is pleased to announce that we have received National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) recognition.
"The NCQA Patient-Centered Medical Home is a model of 21st century primary care that combines access, teamwork and technology to deliver quality care and improve health,” states NCQA President Margaret E. O’Kane. “NCQA’s PCMH 2011 Recognition shows that Delmar Pediatrics has tools, systems and resources to provide their patients with the right care at the right time.”
Medical Homes provide timely, accessible and comprehensive care within the office and coordinate care outside the office. Our team of providers synthesizes all aspects of patient care including monitoring medication usage, interpreting laboratory and imaging results, and tracking visits to outside providers. We rely on families to ensure that we have updated clinical information to optimize care. We are the central resource for all of your child’s healthcare needs.
